At Infinity Rehab, building trusted partnerships with our communities is more than a mission, it’s daily practice. One of our newest collaborations, with Mennonite Village in Albany, Oregon, is already yielding positive results thanks to strong leadership, open communication, and shared goals for quality care and operational efficiency.
Leading the charge is Michelle Kilborn, OTD, MOT, OTR/L and Director of Rehabilitation (DOR) at Mennonite Village, whose impact has been felt across the skilled nursing facility (SNF) campus and beyond.

Beth didn’t hesitate to share high praise for Michelle.

Focused on Solutions and Shared Success
What sets this partnership apart is the proactive approach Michelle brings to therapy services. During conversations with Beth, Michelle offered valuable guidance on how to manage referral-related costs, particularly for occupational therapy (OT). Rather than waiting to be asked, she took the initiative.
Michelle is actively exploring telehealth to make the most of her time outside the building, is preparing to take on a second location, and is working to bring team member Cyndi in more often—all to reduce cost pressures and ensure uninterrupted care.
Beth expressed her gratitude for the thoughtful, solutions-oriented support Michelle provided. That kind of forward-thinking leadership is exactly what we strive to deliver at every site.
